With the plate on, the outlet "looked" good.  This was a repair from a bad install by another individual while we were adding undercabinet lights.
I almost left the shovel right there after digging all day to find another bad wiring install done by others.
Luckily the homeowner was never hurt. Another repair found during a remodel. NO EQUIPMENT GROUND ,AND THE BOX WAS HOT due to the improperly installed madison strap holding the box in the wall.

Homeowner said this switch had been "loose for years".  Only the wire behind the switch was keeping it tight to the plate.
Another repair behind unlicensed or unqualified person doing work on the side.  Looked good with the panel cover on though :)
